To enroll your child, you will need a completed enrollment form and a registration fee of $75(VPK students are exempt from the enrollment fee). Florida law also requires that all students must have a Student Physical Examination Form (HRS-H 3040) and Certificate of Immunization (HRS 680) or a certificate of medical/religious expemption on file at the school. These forms are available from your pediatrician. We must receive the original forms before your child begins school.
